Medical blunders can have fatal consequences - but Donald Caine's life was actually saved by one.

The retired policeman, 57, was called in for a swine flu jab unexpectedly after doctors mixed up his notes with his dad, also called Donald, who died in 1989.

But his GP spotted he had high blood pressure, so ran some tests and found he was suffering from a rare bone condition that had left him perilously close to kidney failure.
