Car has GPS and a transmitter that sends the GPS data with an identifier.
This is then picked up by another station then transferred on to the internet. The internet service used to have street level maps but here is an example

And yes Big Brother really is watching. Bare in mind that it is possible to get street level positioning from your mobile phone. So if the police can get permission they can use your phone to tell them where you are. But they'd have to submit a very strong case before they'd get the permission in the first place.
